- Contract of lease (if leased) or Certificate of Title/Tax Declaration of the property/building (if owned)
- A printed Google map of the business location (or sketch)
- The Occupancy Permit
- Community Tax Certificate (CTC or Cedula)
- Public Liability Insurance (for some businesses)
- Location or Zoning Clearance
- Fire Permit (FSIC)
- Sanitary Permit
